This 28-year-old female had suffered a blow to the face as a teenager. She lost her upper left central incisor and the gum and bone ridge of that tooth's region was damaged.
She had the tooth replaced by a bridge. A bridge is a dental appliance that replaces one or more teeth. On each side of the false tooth, one or more crowns (caps) are attached. The crowns sleeve over adjacent teeth that have been shaved into stumps. The bridge is cemented over the shaved down teeth.
The lost tooth's replacement is misshaped due to the gum/bone ridge deformity caused by the accident. This is a bit unsightly and the patient objects to it. Also, one can see the connection of the false tooth to the adjacent crowns -- which further detracts from the desired natural appearance of the bridge. This is a problem inherent to bridges.
The bridge was removed and the false tooth was replaced via an implant-supported-crown. Also, the teeth to each side of the dental implant tooth received new crowns.
In order to place the dental implant, and to have the area not look deformed, it was necessary to construct new bone and gum in the area that had been damaged. This was performed prior to the placement of the dental implant.

In order to have dental implants to replace one or more missing teeth you may have to undergo a dental and x-ray examination as well as a health history review. Dental implants are metal anchors that act as tooth root substitutes. They are surgically placed into the jawbone. Small posts are then attached to the implant which protrude through the gums. These posts provide stable anchors for artificial replacement teeth.
For most patients, the placement of dental implants involves two surgical procedures. First, implants are placed within your jawbone. For the first three to six months following surgery, the implants are beneath the surface of the gums gradually bonding with the jawbone. You should be able to wear temporary dentures and eat a soft diet during this time. At the same time, your restorative dentist designs the final bridgework or denture, which will ultimately improve both function and aesthetics.
After the dental implant has bonded to the jawbone, the second phase begins. Dr. Rose will uncover the dental implants and attach a small healing collar. After two weeks your general dentist will be able to start making your new teeth. An impression must be taken. Then posts or attachments can be connected to the dental implants. The teeth replacements are then made over the posts or attachments. The entire procedure usually takes six to eight months. Most patients do not experience any disruption in their daily life.
This patient was a 37-year-old female who had continued problems with her front tooth after having fractured it. The tooth received Root Canal Therapy, however, it was not successful. Numerous re-treatments of the root canal proved unsuccessful. Some of the re-treatments were surgical in nature. The surgical procedures, and frequent abscesses, destroy the bone that originally surrounded the tooth's root.
A treatment plan consisting of an implant-supported-crown was chosen.
The bad tooth was removed and the damaged jawbone and gum of the area was reconstructed. Eventually, a dental implant and crown were made to replace the lost tooth.
